If you are insured by CareFirst or another Blue Cross/Blue Shield company, I will need both your date of birth and your insurance subscriber ID (including any letters that precede the number). If you do not provide the data I will not be able to verify your eligibility, and therefore will not be able to get back to you to set up our first appointment. If you are not a Blue Cross subscriber, please indicate which insurance company you will be using. Use the box below labelled "Your Message" to provide this information.
